Interstitial lung diseases (ILDs) are a group of disorders that affect the interstitial tissue of the lungs, leading to thickening of the alveolar walls, impaired gas exchange, and the development of respiratory failure. These conditions may progress and result in pulmonary fibrosis, making timely diagnosis and treatment critically important.

When is treatment for interstitial lung diseases required?

  • Persistent shortness of breath that gradually worsens over time.
  • Chronic dry cough without an obvious cause.
  • Chest tightness, fatigue, and general weakness.
  • Structural lung changes detected on chest X-ray or high-resolution CT scan.
  • Presence of occupational exposure, autoimmune disorders, or idiopathic lung disease.

How is treatment provided?

  1. Diagnosis and pulmonologist consultation – includes chest X-ray, high-resolution CT (HRCT), spirometry, and laboratory testing.
  2. Anti-inflammatory therapy – glucocorticosteroids and immunosuppressive agents are used to reduce inflammation.
  3. Antifibrotic therapy – in idiopathic pulmonary fibrosis, medications are prescribed to slow disease progression.
  4. Oxygen therapy – supplemental oxygen is provided in cases of significant respiratory impairment.
  5. Physiotherapy and pulmonary rehabilitation – specialized breathing exercises help improve lung ventilation and physical endurance.

Main treatment methods

  • Glucocorticosteroids (GCS) – suppress inflammation and help slow disease progression.
  • Antifibrotic medications – reduce the progression of pulmonary fibrosis and improve gas exchange.
  • Immunosuppressive therapy – used in autoimmune-related forms of ILD.
  • Oxygen therapy – improves blood oxygen saturation and reduces shortness of breath.
  • Lung transplantation – considered in advanced, irreversible stages of disease.
