An anal fistula (pararectal fistula) is a pathological tract connecting the rectum with the skin or surrounding tissues, resulting from chronic inflammation and suppuration. The condition is associated with purulent discharge, pain, itching, and discomfort in the anal area and, in most cases, requires surgical treatment.

When is treatment of an anal fistula required?

  • Persistent purulent discharge from the perianal region.
  • Pain, inflammation, or induration of tissues along the fistula tract.
  • Itching, irritation, and discomfort in the perianal area.
  • Recurrent abscess formation accompanied by fever.
  • Impairment of normal defecation and reduced quality of life.

Methods of anal fistula treatment

Conservative therapy (early or preparatory stage)

  • Antibacterial therapy to control infection and inflammation.
  • Topical anti-inflammatory and wound-healing ointments.
  • Physiotherapy (laser therapy, magnetotherapy) as adjunctive treatment.
  • Regular warm sitz baths with antiseptic solutions.

Surgical treatment (mainstay of therapy)

  • Radical fistulectomy or fistulotomy – complete excision or opening of the fistulous tract with subsequent wound healing.
  • Laser fistula ablation – a minimally invasive technique that thermally closes the fistula tract.
  • Seton placement – ligation with drainage using a seton technique, applied in complex or high fistulas.
  • Fibrin glue or biological sealant closure – filling of the fistula tract with a biocompatible material to promote closure.

  • Antibacterial therapy to prevent postoperative infection.
  • Dietary modification to normalize bowel movements.
  • Careful perianal hygiene and use of antiseptic agents.
  • Restriction of physical activity during the first postoperative weeks.
