Treatment of otitis media with exudation

Otitis media with effusion (exudative otitis) is an inflammatory disease of the middle ear in which fluid (exudate) accumulates in the tympanic cavity without pronounced pain. The condition develops due to impaired ventilation of the middle ear, often associated with viral infections, adenoids, allergies, and chronic diseases of the nasopharynx. Without proper treatment, otitis media with effusion may lead to persistent hearing loss and structural changes in the middle ear.

When is treatment for otitis media with effusion required?

  • Reduced hearing, a feeling of ear fullness, or autophony (echoing of one’s own voice in the ear).
  • If symptoms persist for more than 3 weeks and do not resolve spontaneously.
  • Presence of chronic nasopharyngeal conditions (adenoids, sinusitis, rhinitis, deviated nasal septum).
  • Progression to a chronic form with a risk of permanent hearing impairment.

Methods of treatment for otitis media with effusion

Conservative therapy

  • Anti-inflammatory medications – reduce mucosal edema and improve ventilation of the middle ear.
  • Mucolytics (acetylcysteine, carbocysteine) – liquefy the exudate and facilitate its drainage.
  • Antihistamines – indicated when the condition has an allergic component.
  • Vasoconstrictive nasal drops – improve patency of the Eustachian tube.
  • Eustachian tube inflation and catheterization – physiotherapeutic methods aimed at restoring middle ear ventilation.

Physiotherapy

  • Electrophoresis and ultrasound therapy – promote resorption of middle ear effusion.
  • Laser therapy – reduces inflammation and improves local blood circulation.
  • Hyperbaric oxygen therapy (HBOT) – enhances tissue oxygenation and supports regeneration.

Surgical treatment

  • Tympanocentesis – evacuation of the effusion through a puncture of the tympanic membrane.
  • Tympanostomy (ventilation tube insertion) – placement of a ventilation tube for prolonged drainage and prevention of recurrence.
  • Adenoidectomy – performed if enlarged adenoids obstruct the Eustachian tube.
