Pelvic osteotomy is a surgical procedure aimed at correcting deformities of the hip joint caused by congenital abnormalities, hip dysplasia, or post-traumatic changes. The operation restores the anatomical orientation of the pelvic bones, improves joint stability, and helps prevent the early development of osteoarthritis.

Indications for pelvic osteotomy in children

Preoperative diagnostic assessment

  1. Hip joint X-ray. Assesses the degree of dysplasia and bony deformity.
  2. MRI and CT. Provide detailed visualization of bone and cartilage structures.
  3. Joint ultrasound. Evaluates soft tissues and the degree of femoral head decentration.
  4. Orthopedic clinical examination. Analysis of gait, joint range of motion, and femoral neck angles.

Types of pelvic osteotomy in pediatric patients

  • Salter innominate osteotomy. Commonly used for correction of dysplasia in children older than 18 months.
  • Periacetabular osteotomy (Ganz osteotomy). Allows reorientation of the acetabulum to improve femoral head coverage.
  • Double or triple pelvic osteotomy. Indicated for severe deformities requiring extensive correction.
  • Pemberton pericapsular osteotomy. Used in children with pronounced dysplastic changes of the acetabulum.

How the operation is performed

  1. Preoperative preparation. The child undergoes comprehensive assessment and an individualized surgical plan is developed.
  2. Surgical correction. The surgeon performs controlled bone cuts, repositions the pelvic segment, and fixes it with plates or screws.
  3. Postoperative care. The limb is immobilized with a cast or orthosis, followed by a structured rehabilitation program.

Rehabilitation after pelvic osteotomy

  • First 6 weeks: protective regimen with restricted weight-bearing.
  • Physiotherapy, massage, and therapeutic exercise (physical therapy).
  • Gradual restoration of mobility and functional loading.
  • Follow-up radiographic imaging to monitor bone healing.
