Shoulder joint replacement (shoulder arthroplasty) is a surgical procedure aimed at replacing a damaged shoulder joint with an artificial implant. The operation is performed to relieve pain, restore mobility, and improve shoulder function, particularly in cases of diseases or injuries that do not respond to conservative treatment.

Indications for surgery

  • Shoulder osteoarthritis causing destruction of the joint surfaces.
  • Rheumatoid arthritis accompanied by chronic inflammation and joint damage.
  • Post-traumatic arthritis developing after dislocations, fractures, or other shoulder injuries.
  • Avascular necrosis of the humeral head leading to bone collapse.
  • Complex shoulder fractures that cannot be effectively reconstructed without joint replacement.

Types of shoulder joint replacement

  • Total shoulder arthroplasty: replacement of both the humeral head and the glenoid (socket) of the scapula.
  • Hemiarthroplasty: replacement of the humeral head only, with preservation of the native glenoid.
  • Reverse shoulder arthroplasty: used in cases of severe rotator cuff deficiency; the anatomy of the joint is reversed to improve biomechanics and shoulder function.

How the procedure is performed

Preoperative preparation: before surgery, the patient undergoes diagnostic evaluation including X-ray, CT, or MRI, as well as blood and urine tests. An individualized surgical plan is developed.

Anesthesia: the procedure is performed under general anesthesia or regional nerve block, providing effective pain control of the upper limb.

Surgical procedure:

  • Damaged joint surfaces are removed.
  • The shoulder implant is inserted using cemented or cementless fixation techniques.
  • Stability and range of motion of the new joint are carefully assessed intraoperatively.

Completion of surgery: the incision is closed with sutures, and a dressing or orthopedic brace is applied to protect and stabilize the operated shoulder.

Postoperative rehabilitation

Rehabilitation plays a crucial role in restoring shoulder function.

  • During the first weeks, the shoulder is immobilized with an orthosis, and load on the joint is minimized.
  • Within a few days, patients begin passive movements under the supervision of a rehabilitation specialist.
  • Physical therapy focuses on restoring range of motion, strengthening muscles, and improving coordination.
  • Full functional recovery typically takes between 3 and 6 months.
