Surgical treatment of glaucoma

Glaucoma is a chronic disease characterized by elevated intraocular pressure (IOP) and progressive damage to the optic nerve, which can lead to irreversible vision loss. When medical therapy and laser treatments fail to achieve adequate IOP control, surgical intervention is indicated to restore normal aqueous humor outflow and reduce intraocular pressure.

When is surgical treatment of glaucoma required?

  • Progressive glaucoma despite regular use of antiglaucoma eye drops.
  • Persistently elevated intraocular pressure threatening optic nerve damage.
  • Worsening visual fields or decreasing visual acuity.
  • Cases of angle-closure glaucoma when conservative and laser methods are ineffective.

Types of surgical treatment for glaucoma

  • Trabeculectomy. Creation of a new pathway for aqueous humor outflow, resulting in a sustained reduction of intraocular pressure.
  • Deep non-penetrating sclerectomy (DNPS). Partial removal of trabecular tissue without full penetration of the eye, reducing the risk of postoperative complications.
  • Implantation of drainage devices. Placement of microshunts or drainage implants to provide long-term control of intraocular pressure.
  • Cyclophotocoagulation. Laser treatment of the ciliary body to reduce the production of aqueous humor.

How is the surgery performed?

  1. Preoperative evaluation and preparation. The patient undergoes tonometry, optical coherence tomography (OCT), and visual field testing.
  2. Surgical intervention. Depending on the chosen technique, a new outflow pathway is created or aqueous humor production is reduced.
  3. Postoperative care. Anti-inflammatory and healing medications are prescribed, and intraocular pressure is closely monitored.

Recovery after glaucoma surgery

  • Reduced visual and physical strain is recommended during the first weeks after surgery.
  • Regular use of prescribed eye drops to prevent inflammation and promote healing.
  • Scheduled follow-up visits with an ophthalmologist to monitor intraocular pressure.
