Cerclage with extrascleral filling
Cerclage with extrascleral filling

Scleral buckling with encircling band (cerclage with extrascleral plombage) is a surgical method for the treatment of retinal detachment, in which a special silicone element (scleral buckle or encircling band) is placed on the outer surface of the sclera. This creates controlled indentation of the eye wall, allowing the detached retina to reattach to the underlying tissues. The procedure aims to close retinal breaks, relieve vitreoretinal traction, and prevent further progression of the disease.

When is scleral buckling with encircling band indicated?

  • Retinal detachment caused by retinal breaks and vitreous traction.
  • Degenerative retinal changes predisposing to retinal detachment.
  • Retinal tears associated with subretinal fluid accumulation.
  • Situations where there is a high risk of permanent vision loss without surgical intervention.

How is the surgery performed?

  1. Preoperative assessment and preparation. Ophthalmoscopy, ocular ultrasound, and optical coherence tomography (OCT) are performed.
  2. Placement of the buckle. A silicone buckle or encircling band is fixed to the sclera to create external support.
  3. Retinal reattachment. Retinal breaks are treated, and subretinal fluid is drained if necessary.
  4. Postoperative follow-up. The patient is prescribed eye drops and undergoes regular ophthalmic monitoring.

Types of surgical treatment for retinal detachment

  • Localized scleral buckling. Used for single or limited retinal breaks.
  • Encircling scleral buckle (cerclage). A circumferential silicone band is placed around the globe to reduce vitreoretinal traction.
  • Combined techniques. Scleral buckling combined with vitrectomy and laser photocoagulation of the retina.

Recovery after surgery

  • Temporary reduction in visual acuity may occur during the first weeks, followed by gradual stabilization.
  • Physical activity and sudden movements should be limited during the early postoperative period.
  • Regular follow-up visits are essential to monitor retinal status and surgical outcomes.

Advantages of modern retinal surgery in Belarus

  • High effectiveness when performed in a timely manner, allowing vision preservation.
  • Low tissue trauma due to microsurgical techniques.
  • Use of modern biocompatible materials ensures long-term anatomical stability.

In Belarus, scleral buckling with encircling band is performed in modern ophthalmology centers equipped with advanced microsurgical technology. Experienced retinal surgeons apply tissue-sparing techniques that help restore the anatomical position of the retina and preserve visual function.
