Leukemia is a malignant disease that originates in blood-forming tissues, most commonly affecting white blood cells (leukocytes). Leukemia disrupts normal hematopoiesis and is often accompanied by anemia, increased susceptibility to infections, and bleeding tendencies. Treatment of leukemia requires a comprehensive approach that may include chemotherapy, immunotherapy, bone marrow (hematopoietic stem cell) transplantation, and other methods depending on the leukemia subtype and disease stage.

When is leukemia treatment required?

  • When leukemia is diagnosed based on blood tests and bone marrow examination.
  • If the patient develops symptoms such as fatigue, frequent infections, bleeding, decreased hemoglobin levels (anemia), or enlarged lymph nodes.
  • When leukemia cannot be adequately controlled and aggressive treatment is necessary to slow or stop disease progression.
  • If there is a risk of disease relapse after previous treatment and continued therapy is required to prevent recurrence.

Methods of leukemia treatment

Chemotherapy

  • Chemotherapy is the mainstay of leukemia treatment. It involves the use of cytotoxic drugs that destroy abnormal blood cells. Chemotherapy may be administered at different stages of the disease, including as induction therapy or as part of conditioning before bone marrow transplantation, as well as consolidation or maintenance therapy afterward.
  • Medications such as methotrexate, cyclophosphamide, and vincristine are commonly used to suppress the growth of leukemic cells.

Immunotherapy

  • Immunotherapy involves the use of agents that activate or modulate the immune system to target cancer cells. For example, monoclonal antibodies may be directed against specific antigens on the surface of leukemic cells, inhibiting their growth and survival.
  • In selected cases, monoclonal antibody–based therapies and other immune-directed approaches are used to eliminate leukemia cells.

Bone marrow (hematopoietic stem cell) transplantation

  • Stem cell transplantation is used when chemotherapy alone is insufficient or in cases of disease relapse.
  • Transplantation may involve autologous stem cells (from the patient) or allogeneic stem cells (from a compatible donor).

Treatment of systemic involvement and supportive care

  • If leukemia leads to systemic complications or organ involvement, supportive and palliative treatment is used to improve quality of life. This may include pain management and psychological support.
  • Supportive therapy also includes transfusions, infection prophylaxis, and measures aimed at restoring normal immune and hematologic function.
