Cancer of the duodenum is a malignant tumor that develops in the first part of the small intestine, located between the stomach and the jejunum. This disease is most often diagnosed at advanced stages, as its symptoms may remain nonspecific for a long time. Treatment of duodenal cancer requires a comprehensive, multidisciplinary approach and may include surgical intervention, chemotherapy, radiotherapy, and other methods depending on the stage of the disease and the patient’s overall condition.

When is treatment for duodenal cancer required?

  • When a tumor is detected at an early stage using diagnostic methods such as endoscopy, CT, or MRI.
  • When symptoms appear, including abdominal pain, loss of appetite, nausea, vomiting, weight loss, or blood in the stool.
  • When the tumor spreads to neighboring organs such as the stomach, pancreas, or intestines.
  • When specialized oncological treatment is required because conservative or symptomatic therapy does not control the disease.

Methods of treatment for duodenal cancer

Surgical treatment

  1. Tumor resection — a surgical procedure in which part of the duodenum or the entire affected segment is removed if the tumor involves a large portion of the organ. In some cases, adjacent tissues and regional lymph nodes are also removed if metastases are suspected.
  2. Gastrectomy — partial removal of the stomach, performed if the tumor has extended into the gastric region and requires a more extensive surgical approach.
  3. Intestinal reconstruction — after removal of part of the intestine, reconstructive surgery is performed to restore normal passage of food through the digestive tract. This may involve creating an anastomosis between the remaining sections of the intestine.

Chemotherapy

  • Chemotherapy is used to destroy cancer cells and prevent tumor spread. It may be administered before surgery (to reduce tumor size) or after surgery (to eliminate residual malignant cells).
  • Treatment includes the use of cytotoxic drugs and other chemotherapeutic agents that target rapidly dividing cancer cells.

Radiotherapy

  • Radiotherapy may be used alone or in combination with chemotherapy to slow tumor growth, relieve symptoms, or reduce tumor size prior to surgery. This is particularly important when the tumor involves nearby organs such as the pancreas or stomach.
  • Radiation therapy may also be used to treat metastatic disease if the cancer has spread to other parts of the body.

Targeted therapy and immunotherapy

  • In selected cases, targeted therapy may be used to block specific molecular pathways that promote cancer cell growth. These drugs can help slow disease progression or reduce the risk of recurrence.
  • Immunotherapy is used to stimulate the patient’s immune system to fight the tumor and may be beneficial in advanced stages or in cases resistant to standard treatments.

  • If persistent abdominal pain, digestive problems, nausea, vomiting, or loss of appetite occur.
  • If risk factors are present, such as smoking, chronic gastrointestinal diseases, or a family history of cancer.
  • If duodenal cancer is suspected, early diagnostic evaluation is essential to initiate treatment at an early stage.
