Treatment of spinal stenosis

Spinal canal stenosis is a condition characterized by narrowing of the central spinal canal, leading to compression of the spinal cord or nerve roots. The disease may cause pain, weakness, numbness of the limbs, and impaired motor function. Treatment is aimed at reducing symptoms, improving quality of life, and preventing complications.

Main treatment approaches

  1. Conservative treatment. Pharmacological therapy. Physiotherapy and therapeutic exercise (Physical Therapy, PT).
  2. Minimally invasive methods. Epidural injections.
  3. Surgical treatment. Indicated in cases of severe compression or progressive symptoms.
  4. Rehabilitation and prevention. Restoration of motor function and prevention of recurrence.

Conservative treatment

Pharmacological therapy:

  1. Nonsteroidal anti-inflammatory drugs (NSAIDs).
  2. Muscle relaxants.
  3. Analgesics.
  4. Corticosteroids.
  5. Medications to improve blood circulation.
  6. B-complex vitamins.

Physiotherapy:

  1. Electrophoresis with analgesic agents. Facilitates targeted drug delivery to the affected area.
  2. Laser therapy. Stimulates tissue regeneration.
  3. Ultrasound therapy. Reduces muscle spasm and improves blood flow.

Therapeutic exercise (Physical Therapy)

Goals:

  • Strengthening of back muscles.
  • Improvement of spinal flexibility.
  • Reduction of compression on neural structures.

Minimally invasive methods

  1. Epidural corticosteroid injections. Reduce inflammation, edema, and pain.
  2. Radiofrequency ablation. Disruption of pain-transmitting nerve pathways.
  3. Percutaneous decompression techniques. Use of catheters to relieve spinal canal compression.

Surgical treatment

Indications:

  • Progressive neurological deficits (numbness, limb weakness).
  • Loss of bladder or bowel control.
  • Severe pain refractory to conservative therapy.

Main surgical methods:

  1. Laminectomy. Removal of part of the vertebral lamina to enlarge the spinal canal.
  2. Foraminotomy. Enlargement of the intervertebral foramen to decompress the nerve root.
  3. Microdiscectomy. Removal of a portion of the intervertebral disc compressing a nerve.
  4. Spinal stabilization. Implantation of metal instrumentation to prevent postoperative instability after decompression.

Rehabilitation after treatment

  1. Physical activity. Gradual restoration of mobility. Regular physical therapy exercises.
  2. Physiotherapy. Reduction of residual inflammation and acceleration of healing.
  3. Massage. Muscle relaxation and improvement of blood circulation.
  4. Body weight control. Reduction of mechanical load on the spine.

Prevention of spinal canal stenosis

  1. Active lifestyle. Regular physical exercises to strengthen back muscles.
  2. Body weight control. Reduction of excessive spinal load.
  3. Posture. Use of ergonomic furniture and correct positioning when working at a computer.
  4. Timely treatment of spinal disorders. Including osteochondrosis and intervertebral disc herniation.
  5. Avoidance of overload. Limitation of heavy lifting and sudden movements.
