Muscular dystonia is a neurological disorder characterized by involuntary, prolonged muscle contractions that lead to abnormal postures, movements, or tremor. The condition may be focal (affecting a single body area) or generalized (involving multiple body regions). Treatment is aimed at reducing symptoms, improving the patient’s quality of life, and preventing complications.

Main approaches to treatment

  1. Pharmacological therapy. Reduction of muscle tone and control of symptoms.
  2. Botulinum toxin injections. Used to eliminate localized muscle hyperactivity.
  3. Physiotherapy and therapeutic exercise (physical therapy). Improvement of coordination and muscle control.
  4. Surgical treatment. Neurostimulation or destruction of specific neural structures in severe forms.

Pharmacological therapy

Main groups of medications:

  1. Anticholinergic drugs.
  2. Dopaminergic medications.
  3. Muscle relaxants.
  4. Benzodiazepines.

Botulinum toxin injections

The method involves injecting botulinum toxin into hyperactive muscles to achieve temporary relaxation. The main advantages are high effectiveness in focal forms of dystonia (for example, cervical dystonia or blepharospasm) and symptom improvement lasting 3–6 months. Possible side effects include mild muscle weakness and local discomfort at the injection site.

Physiotherapy and therapeutic exercise

Goals:

  • Improvement of coordination and posture.
  • Reduction of muscle tension.
  • Increase in overall mobility.

Methods:

  • Stretching and relaxation exercises.
  • Swimming and aquatic therapy.
  • Fine motor skill training (especially in cases involving the hands or face).

Physiotherapy modalities include ultrasound therapy, electrical stimulation, and heat therapy to reduce muscle spasms.

Surgical treatment

Applied in severe forms that do not respond to pharmacological therapy.

  • Deep Brain Stimulation (DBS) — electrodes are implanted into the basal ganglia (brain structures responsible for movement). Regular stimulation improves motor control.
  • Advantages: reversible effect; effective in generalized dystonia.
  • Limitations: high cost of the procedure; need for long-term follow-up.
  • Selective denervation — surgery on nerve structures to eliminate signals causing muscle spasms.
  • Rhizotomy — transection of a nerve responsible for involuntary muscle contractions.

Prognosis and prevention

In most patients, symptom severity can be reduced and quality of life significantly improved. In severe cases, surgical methods may lead to substantial clinical benefits.

Prevention

  1. Early diagnosis and timely initiation of treatment.
  2. Regular performance of prescribed exercises.
  3. Stress management and control.
